HOOF WOOF MEOW ANIMAL RESCUE VOLUNTEER Waiver and Release of Liability. Hoof Woof Meow Animal Rescue (also known as “HWMAR”) is a non-profit 501C-3, organized and existing under the laws of the State of Illinois and each of its directors, officers, employees, volunteers, and agents. I declare that I am over 18 years of age and desire to work as a volunteer for HWMAR and engage in activities related to being a volunteer. I understand that my participation in any activity is entirely voluntary and that I am under no obligation to engage in any activity or action that I find objectionable, for any reason, as determined in my sole discretion. ● I agree to conduct myself in a courteous and professional manner as a volunteer and as a representative of Hoof Woof Meow Animal Rescue ● I agree to abide by all Hoof Woof Meow animal rescue policies and procedures. ● I understand that it is my responsibility to handle all animals with respect and care and to ensure I utilize all proper handling tools available at all times. ● I hereby allow Hoof Woof Meow Animal Rescue to use any photographs taken of me on property or at a special event for public relation purposes. ● I agree that my services are provided on a volunteer basis without pay or compensation of any kind and all services are to be performed at my own risk. ● I understand that in handling animals and performing other volunteer’s task there does exist a risk of injury including physical harm. I hereby agree that I am providing volunteer services to Hoof Woof Meow Animal Rescue. This may also include: ● attending and assisting at off-site events such as animal meet & greets, or community events to promote HWMAR ● assisting in representing HWMAR on social media by sharing posts and events ● assisting with the transportation of rescue animals ● assisting with handling of the animals at events ● interacting with HWMAR board members, volunteers, fosters, rescue animals and the general public I understand that neither Hoof Woof Meow nor any other businesses related to this offsite activity, is responsible for any illness nor injury caused by any animals that I come into contact with during my volunteer work. I agree to hold harmless and release from liability Hoof Woof & Meow should I become sick or injured from any animals or activities as a result of my volunteer work. I agree that on behalf of myself, my heirs, personal representatives and executors, I release, discharge, indemnify, and hold harmless Hoof Woof Meow its agents, employees, directors and board of directors from any and all claims, causes of action, or demands of any nature of cause, including costs and attorneys fees incurred by Hoof Woof Meow Animal Rescue in connection with the same, based on damages or injuries which may be incurred or sustained by me in any way connected with my services for the Hoof Woof Meow Animal Rescue including but not limited to animal bites, accidents, or injuries. Further, I expressly agree that this release, waiver and indemnity agreement is intended to be as broad and inclusive as permitted by the State of Illinois, and that if any portion of this release is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. I currently have no known mental or physical condition that would impair my capability to provide this release. I agree that I understand the contents and effect of this release, that I have had an opportunity to seek professional advice and that I provide this release willingly.
